Financial assistance program for patients suffering from cancer, kidney, liver cirrhosis, paralyzed by stroke, congenital heart disease and thalassemia

Financial assistance program for patients suffering from cancer, kidney, liver cirrhosis, paralyzed by stroke, congenital heart disease and thalassemia has been launched from the financial year 2013-14. Identifying the poor patients affected by the said disease, with the help of the social services department, local administration, public representatives and well-wishers, following the policies, the list of really poor and helpless people is being implemented. Medical assistance is provided to poor patients suffering from cancer, kidney, liver cirrhosis, paralyzed by stroke, congenital heart disease and thalassemia at the rate of 50,000/- (fifty thousand) per head subject to receipt of budget. Through this, the family of the affected patient is helped to bear the expenses and return to a healthy and normal life.


 


 


Read the following guidelines carefully before applying:


A. The applicant/patient must be a citizen of Bangladesh; Priority will be given to the most distressed person; In terms of financial status: children, destitute, refugees and landless will be prioritized in order; In terms of social status: elderly, widowed, divorced, vulnerable, destitute, separated from family will be prioritized in order; Preference will be given to landless candidates where applicable; In this case, if the amount of land of a person is 0.50 acres or less, apart from the house, he will be considered as landless.

b. A complete application must be submitted to the concerned Upazila Social Service Office along with the following certifications, documents/test reports and information attached to the application:

1. Original copy of relevant disease certificate issued by registered specialist doctor in prescribed form;

2. Patients suffering from Cancer, Kidney, Liver Cirrhosis, Paralyzed by Stroke, Congenital Heart Disease and Thalassemia must submit the following prescription and test report from the specialist doctor for the respective diseases along with the application:

2. 1. Histopathology/Cytopahtology/Bone Marrow Report or other test report in case of cancer;

2. 2. Reports on blood urea and creatinine levels applicable to patients with Acute Renal Failure or Chronic Renal Failure undergoing dialysis, preparing for kidney transplantation or having undergone kidney transplantation;

2. 3. Liver ultrasonogram report and other test report should be available in case of liver cirrhosis;

2. 4. A patient paralyzed by stroke should be certified by a neurologist and MRI/CT Scan Report;

2. 5. Echo Cardiogram report and other test reports in case of congenital heart disease;

In case of thalassemia, there should be a report of blood hemoglobin electrophoresis (Hemoglobin Electrophoresis) or other applicable tests.

3. Photocopy of patient's National Identity Card/Birth Certificate attested by 1st Class Gazetted Officer and 3 recent attested photographs;

4. If the age of the patient is less than 18 years on the date of application / his legal guardian can apply, in that case photocopy of the applicant's national identity card attested by a gazetted officer of 1st class; And 3 copies of attested photographs taken recently.

 


02 (two) copies should be submitted to the respective upazila or city social service office by filling the application form along with the supporting documents and attestation form. Note that applications can be submitted to the said offices at any time of the year. There is no opportunity to send the application directly to the District Social Service Office.

Candidate Selection Process:


1. The Deputy Directors will prepare a preliminary list in the light of the applications received to provide financial assistance to patients suffering from Cancer, Kidney, Liver Cirrhosis, Stroke Paralyzed, Congenital Heart and Thalassemia and submit it to the District Committee.


2. The Deputy Director concerned shall maintain two separate lists and registers containing necessary documents and information regarding the treatment of eligible and ineligible persons primarily among the applicants under his jail for cancer, kidney, liver cirrhosis, paralyzed by stroke, congenital heart and thalassemia patients.


3. The said list and the received applications should be presented in the meeting of the District Implementation Committee and the implementation committee will scrutinize the applications and take measures to distribute the check after approving a single list (with supporting documents) eligible to receive financial assistance.
